The easiest and most convenient method for families is to utilize your My Health Connection Portal. If you are not already connected, please click on the portal link at the top of the page for direction on how to set up your portal. You may also call our office at 970-484-4871, option 2 for scheduling.
We offer online scheduling for the following appointments:
- Well care appointments for ages 3 and up (if your child is under 3 yrs. of age, you will need to call our office for an appointment).
- Sick appointments (daily options available) -if you do not see any availability online, please call our office for additional times available.
- Flu Vaccines
We currently do not offer online scheduling for the following appointments, so please call our office at 970-484-4871 to schedule your appointment.
- New patient visits
- Well care visits for patients under age 3
- Medication checks
- Follow up visits
- Consultations for ADHD, Autism, Asthma, Depression, Developmental concerns, etc.
- Procedures - Circumcision, Wart removal, of Foreign Body removal
- Sports Physicals
- Immunizations - this includes Covid and Beyfortus
